Critical: Observed raw chicken and eggs above vegetables in walk-in cooler, corrected during inspection. Critical: Observed containers holding food that are unclean with old food residual. Observed food in contact with an unclean surface. Observed improper use and/or maintenance of wiping cloths. Observed wet cloth sitting out. Critical: Observed walk-in cooler foods that are temperature controlled for safety (TCS) not date marked. Observed upright residential freezer. Basil’s Restaurant, 18 N. Market St., Troy — Repeat: Observed that condensing unit in walk-in freezer in basement of facility had ice accumulation below and above the unit.
Hobart Arena, Northeast Concession, 255 Adams St., Troy — Critical: Packaged hot dogs in the hot holding unit were observed holding between 101-107 F. To limit microbial growth, all time-temperature control for safety food shall be maintained at or above 135 F. At the time of inspection, the hot dogs were reheated to 170 F. At the time of inspection, an internal food thermometer was not observed.

Observed cardboard used as a base on the shelving unit in storage area to hold bottles of syrup. Observed hand dryer in men’s bathroom that was falling apart and needs to be repaired. Observed temperature dials on warewashing equipment in front bar that were not properly working. Observed cutting board in the food prep area and warewashing area that is heavily scratched. Observed rust and cobwebs underneath cabinet where drink machine behind front bar is located. Repeat: Observed gasket on cooling unit by grills that had food debris build-up. Observed food containers being stacked on rack above three compartment sink after being washed and were not able to properly dry. Observed cups in the front bar area that were use for customers and were not properly washed and had food residual on them. Observed food containers in clean dish section in ware washing are that were not being effectively cleaned and had food residual on them. Observed plates that were cleaned not being inverted when stored behind front bar. Observed storage of utensils where handles were not stored in the same direction behind front bar. Critical: Observed coffee inside of the handwashing sink behind front bar. Observed door to outside dumpster open during inspection. Observed ice accumulation underneath the condensing unit inside of walk-in freezer. Observed missing panel on the side of the condensing unit in walk-in freezer. Observed food debris and trash build-up underneath shelving units in walk-in cooler. Observed dust and debris build-up on the condensing unit in the walk-in cooler. Repeat: Observed debris build-up underneath cooling units, drink dispenser, grills and under shelving in dry storage area. Observed debris build-up on the drain below hand sink in women’s restroom. Observed drain below cooling unit that had residual build-up and was not being properly cleaned. Observed employee medicine and fingernail polish in front food prep area during inspection. Critical: Observed gnats in the warewashing area and in food prep area. Critical: Observed working containers of chemicals being stored with food items in front bar area, corrected during inspection.

Steak N Shake, 1779 W. Main St., Troy — Critical, repeat: Observed build-up of food residual on inside of top plate, sides and the bottom of both microwaves. Repeat: Observed food debris accumulation in the cabinet under the dessert/shake bar in food prep area. Repeat: Observed residual build-up on the handles on the majority on the equipment in food prep area. Repeat: Observed leak coming from the air gap piping behind the hot water heater in the storage area. Repeat: Observed grease accumulation on piping and underneath all fryers near front cook line. Repeat: Observed a build-up of grease on piping behind vent hood cover above ovens in front cook line. Critical, repeat: Observed the presence of live gnats in storage area above the mop sink and at the warewashing station.
The Hershey Bar & Grill & Patio, 118 E. Main St., Bradford — Repeat: Without prior approval, the prep sink and corresponding plumbing was removed while the three compartment sink and corresponding plumbing was relocated. In the back storage area, a noncommercial chest freezer was observed being used to store food. Repeat: In the kitchen, noncommercial produce slicers were observed. Repeat: In the bar area, the bar cooler on the left hand side was observed with pooling water on the bottom surface. Repeat: Several non-food contact surfaces were observed with dirt and dust debris build-up, including the bottom shelf of the prep table adjacent to the hand sink, the bottom shelf of the pizza prep table, the spice storage shelf. Repeat: at the time of inspection, the three compartment sink plumbing was observed in disrepair — not to Ohio building code. Repeat: In the walk-in cooler, the ceiling was observed in disrepair — bowing. Repeat: The ventilation hood system was observed with dust debris and grease build-up.
JJ’s Lunchbox, 7031 S. County Road 25-A, Tipp City — Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. Observed wiping cloth being stored on prep table outside of sanitizing water, corrected during inspection. Observed that facility did not have sanitizing test strips in the facility during inspection. Observed that the cutting boards in the back area were heavily scratched and stained. Observed leak on faucet of handwashing sink and three compartment sink during inspection.
Bakehouse Bread Company, 317 SW Public Square, Troy — Observed miscellaneous items not used in prepping food being stored on wooden food prep table during inspection. Observed that wooden bowls used to hold miscellaneous items in food prep area were not easily cleanable in food prep area. Observed no sanitizing test kit for the three compartment sink in the facility. Observed cutting board on the prep cooler and the sealed wooden food prep surface that had cuts on the surface and need to be resurfaced. Observed dust build-up on the fan casing located next to mixing units in food prep area. Observed dirty aprons hanging on shelving unit where clean food equipment was being stored. Observed dust build-up underneath the cooling unit towards the back of food prep area.
Tipp O’ Town, 1150 W. Main St., Tipp City — Repeat: Observed employee food sitting on shelving unit with other food equipment in storage area. Repeat: Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. Repeat: Observed wiping cloth in the waitress area that was not being stored in the proper concentration of sanitizer. Repeat: Observed handle of walk-in cooler was falling away from the door. Repeat: Cutting board in front of prep grill was stained and scoured severely. Repeat: Observed dust and residual build-up on carbonator for pop machine on shelf where packaged bread is stored. Repeat: Observed handles on equipment throughout facility had residual on them. Repeat: Food holding rack was observed to be stored in the maintenance area during inspection. Repeat: Observed that gasket on the walk-in freezer door was not sealing properly. Repeat: Observed holes in wall in the storage area. Repeat: Observed that the coving on the base of the wall by the exit door and underneath the dishwashing unit was coming apart from the wall. Repeat: Observed dust and grease build-up on the ventilation hood covers above grills. Repeat: Observed food debris on the shelving units in the walk-in freezer. Repeat: Observed grease build-up behind and underneath fryers in cook line. Repeat: Observed dust on the ceiling and walls of food prep area. Repeat: Observed a build-up of food debris on the floor of the walk-in freezer. Repeat: Observed mops in the restroom area that were not drying correctly. Repeat: Observed pots not used being stored on floor inside of storage area. Repeat: Observed multiple unnecessary items inside of the maintenance area.
