Fulton Farm Market, 2393 State Route 202, Troy — No violations were documented at the time of inspection.

Oct. 1o

Troy BK Rootbeer, 2780 Stonecircle Drive, Troy — Critical: Observed shredded cheese being held at 55F by hot holding unit in food prep area. Critical, repeat: Observed Ready-To-Eat cooked hamburger and chicken that was not properly date marked in food prep cooler. Observed handle of the fridge unit by the fryers falling apart. Observed leak on piping underneath three compartment sink in the front area. Critical: Observed working containers of chemicals that were not properly labeled by three compartment sink in back food prep area, corrected during inspection.

Homestead Golf Course Pop House, 5327 Worley Road, Tipp City — A noncommercial microwave was observed in the facility. Repeat: In the corners of the facility, under the cabinets and in other hard-to-reach areas, cob webs and dust build-up were observed.

Oct. 13

Covington K-8 School, 807 Chestnut St., Covington — Observed dented can in back storage area, corrected during inspection. Critical: Observed container of salsa that was to be disposed of on Oct. 11 still in walk-in cooler, corrected during inspection.

The Hershey Bar & Grill & Patio, 118 E. Main St., Bradford — Critical: Observed bare hand contact with two hamburger buns. Critical: Two bottles of homemade zesty sauce and one container of cooked chicken were observed without date marking. Without prior approval, the prep sink and corresponding plumbing was removed while the three compartment sink and corresponding plumbing was relocated. Dry ingredients on the bottom shelf of the pizza prep table were observed being stored in tote storage containers. Repeat: Food was observed being stored in the walk-in cooler, which was not approved for the storage of food items. In the kitchen, noncommercial produce slicers were observed. In the bar area, the bar cooler on the left hand side was observed with pooling water on the bottom surface. The following non-food contact surfaces were observed with dirt and dust debris build-up: the bottom shelf of the prep table adjacent to the hand sink, the bottom shelf of the pizza prep table, the spice storage shelf. In the bar area, the ice scoops were observed being in a container with dust and dirt debris build-up. In the kitchen area, the utensil sorter beside the Pepsi cooler, along with the knife sorter on top of the pizza prep cooler were observed with dust and food debris build-up. At the time of inspection, the three compartment sink plumbing was observed in disrepair – not to Ohio building code. In the walk-in cooler, the ceiling was observed in disrepair, bowing. The ventilation hood system was observed with dust debris and grease build-up.

Kiwanis Club of Troy mobile, PO Box 830, Troy — No violations were documented at the time of inspection.

Oct. 16

Raise Your Brush, 7 S. Market St., Troy — Critical: Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Observed ceiling tiles that were bowing downwards due to water leakage from ceiling.

Steak N Shake, 1779 W. Main St., Troy — Observed employee drink sitting out on food prep sink. Critical: Observed avocado spread (51 F), salsa (55 F) and sliced tomatoes (50 F) not holding proper temperatures inside prep cooler in front cook line, corrected during inspection. Observed prep cooler on front cook line was not working properly as not all foods were to the correct temperature. Critical, repeat: Observed build-up of food residual on inside of top plate, sides and the bottom of both microwaves. Observed food and dust residual on the shelving units inside of walk-in cooler. Repeat: Observed food debris accumulation in the cabinet under the dessert/shake bar in food prep area. Repeat: Observed residual build-up on the handles on the majority on the equipment in food prep area. Observed leak coming from the air gap piping behind the hot water heater in the storage area. Observed loose garbage accumulation on ground around dumpsters. Observed FRP wall bowing outwards right above the top of the mop sink. Observed ice accumulation on the ceiling panel and on the fan covers of the condensing unit of the walk-in freezer. Observed grease accumulation on piping and underneath all fryers near front cook line. Observed a build-up of grease on piping behind vent hood cover above ovens in front cook line. Repeat: observed debris building up underneath equipment throughout food prep area. Critical: Observed the presence of live gnats in storage area above the mop sink and at the warewashing station.

Oct. 17

Sunoco Food Mart, 4455 Gibson Drive, Tipp City — Observed a coke pop machine newly installed that was with new drain pipe system with direct connect going to a mop sink. The plumbing system does not comply with the Ohio building code.
