Arby’s Roast Beef, 1230 E. Ash St., Piqua — The prep unit has water standing in the bottom. Repair. The door on the walk-in cooler was not closing tightly. Repair. Be sure food employer knows to report symptoms of nausea, sore throat with fever, diarrhea and jaundice to person in charge.
Burger King, 1408 Covington Ave., Piqua — The following items need to be cleaned: blender, blender wash station, ice machine, food containers, pans and pitchers. The lid for the hamburger freezer and the ice machine interior seal are both broken. Repair. The fryer area, the exterior of chest freezer and the walk-in fan need to be cleaned.
Red Roof Inn & Suites, 902 Scott Drive, Piqua — Container of gravy cooling in refrigerator. Cooling and reheating of items not permitted with current license. Also using deep container. Discard. Must date mark all ready-to-eat TCS foods, i.e. eggs that are hard boiled. Must discard after seven days. Orange juice container thawing on counter. Do not thaw at room temperature. Thaw in refrigerator or under running, draining water. Provide sanitizer so utensils can be washed, rinsed and sanitized properly. Provide metal stem thermometer so food temperatures can be checked. Provide paper towels to work at hand sink. The food equipment and pans were being put away wet. Allow these items to dry before putting them away. The ceilings and floors need to be cleaned. When using time as a public health control, i.e. tomatoes, cut lettuce, be sure to mark the time the item is removed from refrigeration. Must discard any remaining food after four hours.

DJ’s Pizza Alley, 18 S. Second St., Tipp City — Observed facility did not have a written procedure for bodily fluid clean up.
Family Dollar, 930 W. Main St., Tipp City — Cross connection between the drinking water system and a non-drinking water system. Observed hose attached to mop sink and stored in sink bay, no backflow preventer. Person-in-charge agrees to keep hose out of sink bay. Observed no Level One Certificate present. PIC stated she has a Level One Certification, which is an internal training provided by Family Dollar. The inspector will verify that this internal training is approved by State of Ohio.
Fairfield Inn, 83 Troy Town Drive, Troy — Critical: Waffle mix and hard boiled eggs were observed being maintained at room temperature (63 F – 66 F). Person-in-charge discarded the food items and decided to use time as a public health control for the eggs and waffle mix. Critical: One package of deli ham was observed with a date marking of Sept. 25. Upon discovery, the deli ham was discarded. Critical: Multiple apples in the breakfast area were observed uncovered; corrected.
Dollar Tree, 1885 W. Main St., Troy — Observed facility was without written procedure for cleaning bodily fluids. Observed facility was without Level One Food Safety Trained employee. One is needed each shift at all times facility is open
Walgreens, 20 W. Market St., Troy — Observed no written procedure for cleaning up bodily fluids.
Storypoint Of Troy Bistro Pub, 1840 Towne Park Drive, Troy — Observed dried soda syrup spray in the cabinet under the fountain soda machine and mold growth on the drain line of the ice well in the same cabinet.
Guadalajara’s Mexican Restaurant, 301 E. Troy Pike, Suite C, Covington — Critical: In the prep cooler across from the hard top grill, raw eggs were observed being stored above ready-to-eat food. Also, in the walk-in cooler, raw beef and raw chicken were observed being stored raw pork. In both instances, the food items were moved and properly separated. Critical, repeat: The chemical sanitizing solution in the warewashing machine was observed at 0.0 ppm; corrected. Critical: In the front area, one container of salsa was observed holding at 48 F; corrected. Critical: Multiple food items in the walk-in cooler and prep coolers were observed past their use-by-date; corrected. In the prep sink, raw beef was observed being thawed in standing, warm water. Also, 13 packages of tilapia were observed being thawed under ROP environment. The person-in-charge prepared the beef and the 13 packages of tilapia were discarded. Food storage pans on the wire shelving adjacent to the dishmachine were observed being stored wet.
