Oct. 16

Piqua Marathon, 226 W. Water St., Piqua — Satisfactory at time of inspection.

Actionworks LLC, 8433 N. County Road 25-A, Troy — Provide test strips for sanitizer to monitor amount used. Provide thermometer for bar cooler so temperature can be monitored. The following items need cleaning: tongs by fryer, microwave interior, pizza paddles. Provide paper towel at hand sink. Clean floors in kitchen. Replace missing damaged ceiling tile.

Oct. 17

Susie’s Big Dipper, 323 N. Main St., Piqua — Label all bulk food container to prevent misuse unless unmistakable. Provide ingredients for all food times, i.e. soups, ice cream, granola bars, etc. that are available for customers self service. Remove excess cardboard from basement. Provide paper towels for hand sink in basement kitchen.

Hot Head Burritos, 1567 Covington Ave., Piqua. Bottle of lime juice on counter marked “Keep Refrigerated.” If marked keep refrigerated, keep below 41 degrees. Item discarded. Cracked food containers. Replace.

Oct. 18

Taco Bell, 1429 Covington Ave., Piqua — Food containers and the metal stem thermometer need to be cleaned. Items moved to three-compartment sink. Be sure to wash hands often. Wash after breaks, before putting on clean gloves, etc.

Piqua DDQ Grill and Chill, 1288 E. Ash St., Piqua — Provide thermometer for front prep unit. The lid on the freezer by the fryer is broken. Repair/replace. The hand sink is damaged and is coming off the wall. Repair. The following items need to be cleaned: hot holding cabinet fryer hood exterior, condiment dispensers, sandwich prep unit, chili warmer, grill hood, single door refrigerator, thawing tray in walk in. Be sure food employees wash hands often. Washing before putting on clean gloves, after breaks, after handling raw meats, etc.

Oct. 19

McDonald’s West, 1546 Covington Ave., Piqua — Food containers on clean shelf need to be cleaned. Items moved to dirty area. The following items need to be cleaned: shake machine exterior, grill area, egg area, walk in freezer. Be sure to wash hands often.

Oct. 23

Long John Silvers, 1214 E. Ash St., Piqua — The pop nozzles at the dining room machine need to be cleaned. Cracked/damaged food containers. All food containers must be smooth intact and durable. Replace. There are holes in the celling. Repair.

Oct. 24

Taco Bell, 1234 E. Ash St., Piqua — Observed food containers in need of cleaning. Items stacked to be cleaned. Food containers stacked wet. All to air dry fully before stacking. Be sure to wash hands often.

Oct. 25

Tim Horton’s, 635 W. Water St., Piqua — Observed butter marked “keep refrigerated” on counter at room temperature. If marked keep refrigerated must be kept below 41 degrees Item discarded. The lid onto prep unit was leaked and needs to be replaced.

Oct. 18

Dominos Pizza, 937 W. Main St., Tipp City — Facility does not have a person with level two certification in food protection. Critical: Observed food debris in hand sink by prep top cooler. Person-in-charge stated they use this hand sink to drain cans of vegetables for the prep top cooler. PIC agrees to no longer use hand sink to drain vegetables. Observed faucet of mop sink was loose and the whole sink was not attached to the wall. Repair or replace. Facility did not have level one certified employee present during inspection.

Frisch’s, 20 Troy Town Road, Troy — Critical: In the walk-in cooler, two pans of raw fish were observed being stored above two pans of French toast. Also in the walk-in cooler, a pan of raw hamburger was observed being stored above ready-to-eat food. Upon making person-in-charge aware, the food items were placed in the correct locations. Critical: On the buffet line, the vegetable and potato soups were observed holding between 120 F and 122 F. Upon making person-in-charge aware, the soups were reheated to above 165 F. On the buffet line and then in the cooking area, ice (unapproved cooling mechanism) was observed being used to keep food items cold. Mechanical refrigeration is required. Critical: In different storage areas throughout the kitchen, plastic storage containers were observed cracked and with surfaces missing; discarded. The hot water sanitizing warewashing machine was observed leaking water. Repair or replace.

Oct. 19

Big Mike’s Gas N Go, 8561 State Route 201, Tipp City — No thin probe thermometer available. Critical: Missing and/or incomplete allergen declaration(s).Observed chocolate brownies. Observed no Level One Certificate in facility

Speedway, 4 Weller Drive, Tipp City — Facility does not have a person with level two certification in food protection. Observed hand sink in men’s restroom not able to provide water of at least 100F for hand washing. Critical: TCS foods not being hot held at the proper temperature; discarded.
