Health inspections

Oct. 12

Troy Apostolic Temple Inc., Little Jacobs Ladder, 625 N. County Road 25-A, Troy — Critical: At the time of inspection, a recent water sample was not available. The cooking and serving utensils being stored in the drawer and then the serving spoons being dried were observed scattered and with the handles down. Store the utensils all facing one way with the handles accessible.

Oct. 13

Hobart Arena, main kitchen, 255 Adams St., Troy — The hand sink plumbing between the dirt dish area and ice machine was observed leaking water. Repair or replace.

Oct. 15

Arby’s, 17 Weller Drive, Tipp City — No violations were documented at the time of inspection.

Oct. 16

Action GBW, 201 Staunton Road, Troy — No violations were documented at the time of inspection.

Honeybaked, 771 W. Market St., Troy — Facility does not have a person with level two certification in food protection. Critical: Observed mold growth on the underside of the cutting board used on the prep top cooler; corrected during inspection. Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed cutting boards no longer smooth and easily cleanable

Bowman & Landes Turkeys Inc., 6490 E. Ross Road, New Carlisle — Critical: In the prep cooler, the containers of side items were observed without date markings. Upon discovering this, the side items were date marked.

Oct. 17

Tipp City Road Dog, 1200 W. Main St., Tipp City — Critical: Temperature controlled for safety foods not being hot held at the proper temperature. Observed chicken on a stick holding in the hot holding cabinet at 125F. Also, observed salt water solution used to brine raw chicken before frying holding outside of temperature control at 73F. No written procedure for time as a public health control or time stamps were present for either item. Person in charge stated they will use time as a public health control for chicken on a stick and salt water brine. Written procedure in place and time stamps now on both products. Observed overflow pan under water heater full of water. Eliminate source of leak

Oct. 18

Papa John’s, 779 W. Market St., Troy — Repeat: Facility does not have a person with level two certification in food protection. Repeat: Hand washing sink water below 100F. Employee restroom hand sink not able to provide water of 100F. Critical, repeat: Observed large number of drain flies/fruit flies around prep sink in back prep area besides three-compartment sink. Repeat: Water source and/or hot water capacity not sufficient. Observed hand sink in employee restroom not able to provide water of 100F. Repeat: Observed missing floor tiles and tile base under three-compartment sink around newly installed grease trap.

Hickory River Smokehouse, 135 S. Garber Drive, Tipp City — Critical, repeat: The hot water sanitizing ware washing machine final rinse temperature was observed at 156 F. Upon observing this, the person-in-charge ceased the use of the dish machine to sanitize and used the three compartment sink. Repeat: The front kitchen area reach-in freezer was observed with ice build-up on the bottom surface. Repair or replace. Repeat: The hot holding unit was observed with a gap between the door and door frame. Repair or replace.

Garbry Ridge, 1567 Garbry Road, Piqua — Satisfactory at time of inspection.

Sterling House of Piqua, 1744 W. High St., Piqua — Provide test strips or maximum registering thermometer for high temp dish washer.

Subway, 1300 E. Ash St., Piqua — The pop nozzles and vegetable slicer need to be cleaned.
