Oct. 10

Lucky’s Pad, 108-110 N. Main St., Piqua — Observed signs of mice. Increase level of sanitation and continue use of traps.

Pizza Hut, 1631 Covington Ave., Piqua. Observed following items that need to be cleaned: pizza rings, metal stem thermometer, can opener, dish warmer rack.

Oct. 11

Rally’s, 118 N. College St., Piqua — Tomatoes out past four hours. Discard after being out four hours. Corrected. Observed gnats in front ice cream machine bin. Discard. The interior of the microwave and food containers by three-compartment sink need cleaning. The interior of freezers need cleaning. Floors and ceilings need to be cleaned. There is a smell coming from mop sink. Repair. Ensure food is protected from contamination.

American Legion Post 184, 301 W. Water St., Piqua — The hoods need to be cleaned. Observed bottle of cleaner stored by food equipment. Keep chemicals away from equipment to prevent contamination. Corrected.

Oct. 7

Homestead Golf Course Pop House, 5327 Worley Road, Tipp City — At the time of inspection, the establishment was without an individual who had level two food safety certification. Critical: At the time of inspection, the establishment was without an employee health policy and employees were not informed of their responsibility. At the time of inspection, the establishment was without a bodily fluid clean-up procedure. Obtain a procedure. Critical: At the time of inspection, the establishment was observed without an EPA regulated sanitizer; corrected during inspection. Critical, repeat: At the time of inspection, the person-in-charge could not provide a recent water system sample. Prior to opening next season, provide a water sample.

Oct. 9

Action GBW, 201 Staunton Road, Troy — Observed rodent droppings under chip rack by exit garage door of drive through area.

Tippecanoe High School, 615 E. Kessler Cowlesville Road, Tipp City — No violations were documented at the time of inspection.

La Quinta Inn & Suites, 19 Weller Drive, Tipp City — Repeat: Establishment was without an individual with level two food safety certification.

Holiday Inn Express, 60 Troy Town Drive, Troy — Critical, repeat: One pan of sausage gravy on the buffet was observed holding at 108 F. Upon observing this, the pan of sausage gravy was discarded. Repeat: Equipment not approved by a recognized testing agency.Observed noncommercial crock pot in use in kitchen and noncommercial can opener in use in kitchen.

Wendy’s, 1300 Archer Drive, Troy — No violations were documented at the time of inspection.

The Hershey Bar & Grill & Patio, 118 E. Main St., Bradford — Repeat: The commercial chest freezer lid was observed in disrepair. Repeat: In the back area and side dry good storage area, mouse droppings were observed. Clean the mouse droppings, and if mouse droppings return after cleaning, consult license exterminator.

Frisch’s Restaurant, 20 Troy Town Road, Troy — At the time of inspection, dead bed bugs were observed in the holed out area of the office desk. Remove.

Railroad Restaurant, 629 S. Crawford St., Troy — Repeat: The back kitchen door was observed with a gap between the door and threshold. Repeat: In the storage container adjacent to the back kitchen door, the cooking and serving utensils were observed scattered. Repeat: Directly behind the bar and then in the three-compartment, hand, and prep sinks area exposed wood was observed. Cover or seal. Repeat: In the back kitchen area behind the chest freezers, the hole in the wall was observed. Fill in or cover to prevent pests.
