Oct. 1

Noah’s House Christian, 608 W. High St., Piqua — Observed signs of mice in kitchen. Use traps or other professional treatments to correct problem. Clean thoroughly. Pest control operator has been contacted.

Papa John’s No. 594, 500 N. Main St., Piqua — At least one person in charge per facility must have Level 2 food safety training. Provide training as required. Containers of cheese at room temperature marked “keep refrigerated.” Item discarded. Damaged lid on prep unit. Repair. Damaged and cracked food containers. These must be smooth and intact. Discard. The following items need to be cleaned: boxes with pizza boxes in them, dispensing utensils, fan in walk-on, tomato cutter, container holding food utensils.

Nicklin Avenue Market, 1601 Nicklin Ave., Piqua — Obtain Level 2 food certification. Must wear gloves when handling ready-to-eat foods. Only thaw TCS foods in refrigerator, under running water or while working. Not on counter. The fan in front walk-in in need of cleaning. Replace ceiling tiles by walk-in.

Sept. 25

Broadway Elementary School, 223 W. Broadway St., Tipp City — The hot water sanitizing ware washing machine was observed with a final rinse cycle temperature of 175 F while the plate surface temperature was observed at 161 F (181 F). Have dish machine temperature measuring device calibrated and ensure it is reaching at least 180 F.

Sept. 26

Nevin Coppock Elementary School, 525 N. Hyatt St., Tipp City — The Hobart reach-in cooler door gasket was observed in disrepair. Replace. Repeat: The walk-in freezer fan unit along with surfaces on the corner of the door were observed with ice build-up. Repair or replace.

La Quinta Inn & Suites, 19 Weller Drive, Tipp City — Establishment was without an individual with level two food safety certification. Have an individual obtain certification. Critical: The food items in the Kelvinator reach-in cooler were observed holding between 51 F and 55 F. During the inspection, the food items were discarded. In the kitchen, the Kelvinator reach-in cooler was observed with ice build-up and holding at 51 F. Cease use of the cooler and repair or replace. Back storage area refrigerator, kitchen coffee makers and kitchen microwave were observed non-commercial. Replace with commercial.

The Hershey Bar & Grill & Patio, 118 E. Main St., Bradford — The knife sorter on top of the pizza prep cooler were observed with food debris build-up. Remove the knives and clean. Repeat: The commercial chest freezer lid was observed in disrepair. Repair or replace. The sides of the fryers were observed with food debris and grease build-up. In the back area and side dry good storage area, mouse droppings were observed. Clean the mouse droppings and consult a licensed exterminator.

Holiday Inn Express, 1100 W. Main St., Tipp City — At the time of inspection, the establishment was without an individual certified in level two food safety. Obtain certification.

Oct. 1

Buffalo Jacks, 137 S. High St., Covington — Repeat: In the walk-in freezer, water was observed leaking from the fan unit and corresponding tubes resulting in ice build-up. During the inspection, the food items were moved. Repeat: Throughout the kitchen, the drawers, sorters and storage containers storing multi-use spoons, forks and knives along with cooking and serving utensils were observed with food debris build-up. remove utensils and clean. Repeat: In the drawer on the prep table across from the food cans, cooking and serving utensils were observed scattered. After cleaning the drawer and utensils, place the utensils in one direction with the handles accessible. Repeat: The ice machine lid was observed in disrepair. Repair or replace. Repeat: In the walk-in freezer, ice build-up was observed on the fan unit and adjacent tubes. Repair or replace. Repeat: The following nonfood contact surfaces were observed with dust or food debris build-up: the plate sorter under the front kitchen area soda fountain machine, the interior surfaces of the microwave adjacent to the ice cream freezer, the wire storage shelving units throughout the kitchen, the bottom shelves of the prep tables, the interior surfaces of the reach-in freezer. Repeat: The floors under pieces of equipment and sinks not only in the kitchen but also the bar area were observed with dirt and food debris build-up along. Repeat: The HVAC vents and corresponding ceiling tiles and then the ventilation hood filters were observed with dust and dirt debris build-up.

Oct. 2

Dunaways Beef & Ale, 508 W. Main St., Troy — Repeat: At the time of inspection, the establishment was without an individual who is trained in level two food safety. Critical, repeat: The bar chemical sanitizing ware washing machine was observed with a chlorine sanitizing solution of 0.0 ppm. Repeat: The single-serve utensils on the shelving unit adjacent to the deli slicer were observed scattered. Store the utensils all facing one way with the handles accessible. Repeat: The bottom section of the sandwich prep cooler was observed with pooling water. Repeat: The following nonfood contact surfaces were observed with a black, mold-like substance build up along with dirt and dust debris build-up: the internal surface of the ice machine immediately below the ice machine lid, the wire shelving units in the walk-in cooler, the wire shelving unit adjacent to the deli slicer, the bottom shelf of the table adjacent to the sandwich prep cooler. Repeat: The bar hand washing sink plumbing was observed leaking water. Repair or replace. The kitchen three compartment sink plumbing was observed installed without a plumbing permit. Repeat: The floors under pieces of equipment throughout the kitchen were observed with dirt, dust and food debris build-up. Repeat: Lights in the kitchen area were observed out resulting in less than 50 foot candles of lighting intensity. Replace the broken lights

Wendy’s, 1300 Archer Drive, Troy — Critical: Observed gnats on walls around fountain soda machine in lobby and floors around mop sink in back of kitchen near prep sink. General manager stated they will have ecolab visit the facility in the next week and treat areas listed above where gnats were observed.
