Arby’s, 17 Weller Drive, Tipp City — Critical violation at time of inspection: observed food debris in hard-to-reach areas of meat slicer. Hand out on slicer cleaning was given. Observed dust on small fan in back line. Clean fans more often to prevent build-up. Observed a grease leak from the fryer area. Repair to eliminate leak.
JJ’s Lunchbox, 7031 S. County Road 25A, Tipp City — Previous issues have been resolved. Please caulk the gap between the FRP board within 30 days.
Kyle Elementary School, 519 S. Plum St., Troy — The screen door is ripped and needs replaced.
Meijer Gas Station, 1990 W. Main St., Troy — Satisfactory at time of inspection.
Miami East High School, 3925 N. State Route 589, Casstown — Observed some food debris on slicer in kitchen. Suggest getting small brushes to clean hard-to-reach places to minimize pathogen growth.
Springcreek Primary School, 145 E. U.S. State Route 36, Piqua — Food service in good operation at time of inspection.
The Hershey Bar and Grill, 118 E. Main St., Bradford — Critical violation at time of inspection: observed egg cartons on shelf. Manager states they get the eggs from local farmers. Meat and eggs must be obtained from approved sources. Critical violation at time of inspection: observed pepperoni, sausage, cheese in containers without datemarking. Date mark all TCS foods to limit germ growth. Get a grease pen! Critical violation at time of inspection: observed growth inside ice machine. Clean often to prevent germ growth. The floor tiles in walk-in cooler are still broken and need replaced.
Weldy’s Dairy Bar, 2185 S. Miami Ave., Bradford — Operator is cleaning up kitchen and is closing for season (is closed.)
Forest Elementary School, 413 E. Canal St., Troy — Discussed with manager to keep cheese sticks in refrigerator as long as possible to keep below 41 degrees. Discussed options available.
Foxes Pizza Den, 924 W. Main St., Tipp City — Critical violation at time of inspection: Test strips are needed for the dishmachine (chlorine) and three-tank sink (quat ammonium.) Please obtain for both.
Speedway, 1475 W. Market St., Troy — Observed back section of roller grill not holding roll grill food at 135 degrees or higher. Ensure these hot holding foods maintain 135 degrees or greater to limit bacteria growth. Manager is going to monitor temperature and adjust temperatures properly. Focus on breaded type roller grill items. Observed several flies. Eradicate flies safely. Use fly lights or attractant systems to control the pest. Install in areas that away from food or clean surfaces.
VFW Pleasant Hill, 7578 W. Fenner Road, Pleasant Hill — Walk-in freezer has ice build-up. Fix issue from condensation or warm air leak. Clean ice machine of hard water residual when needed or quarterly.
Arby’s, 963 W. Main St., Troy — Observed prep cooler holding TCS foods at 45 degrees. All TCS foods must maintain 41 degrees or below to limit bacterial growth. Manager is going to monitor temperatures and have issue corrected. Clean hard-to-reach areas, corners, storage areas, freezer flock, under pop machine/ice bins of residual or food build-up. Observed residual build-up. Fix water leak issue under ice bin. Fix water leak at faucet three-compartment sink. Felt food at residual on handles of equipment. Clean frequently. Observed cutting boards that need resurfaced or replaced.
Frisch’s, 20 Troy Town Road, Troy — Previous issues resolved at this time. Observed small leak from pipes in walk-in freezer. Repair. Don’t store any food underneath until it is repaired.
Milton-Union Recreation Association, 7640 Milton-Potsdam Road — Operation good at time of inspection.
Tipp City Eagles, 202 Main Street, Tipp City — Observed two-door upright cooler with condensation water issue. Fix issue properly and remove towels in bottom. Observed a residential refrigerator. All food equipment must be health code compliant. Observed liquid in bottom of beer cooler. Clean out the bottom of the cooler, ensure drain/condensation tube is open.
Troy Junior High School, 556 N. Adams St., Troy — Observed growth inside the ice machine. Clean often to prevent growth.
Burger King, 957 W. Main St., Tipp City — Food service floors need regrouted. Grout lines are full of food oil residual. This will help make floor smooth and easily cleanable. Clean cook line equipment of food residual oil build-up when needed or when required. Observed oil residual in cook line/prep line equipment.
Cookson Elementary School, 921 Mystic Lane, Troy — Discussed with manager the cleaning of the ice machine and how often it should be cleaned. Satisfactory at time of inspection.
Foxes Pizza, 19 N. Miami St., West Milton — Observed the tomato cutter with bent blades and black plastic missing. Replace blades and black holder. The can opener found with food debris. Please clean more often. The coving is still missing in kitchen. Replace coving within two months. REPEAT. The floor tiles are missing in the kitchen. Some are worn to bare wood. Replace tiles within two months. REPEAT.
Subway, 1281 S. Dorset Road, Troy — Observed steak meat thawing in bowl on table. Thawing must be done under cold running water, under refrigeration, during cooking process, or in microwave. Critical violation at time of inspection: worker states the clean knives used are cleaned every four to six hours. Utensils must be cleaned every four hours to prevent germ growth. Handles of equipment are unclean. Wiping buckets should be used and easily accessible to clean handles and counters.
Taco Bell, 1420 W. Main St., Troy — Observed low levels of grout on/between tile floor. Regrout between tiles properly.

Tasty Treat, 129 College St., Piqua — Drain liner for ice cream machines are not properly air gapped. Must be an air gap between drain line and main line to prevent potential back flows. Remove hose from main line.
Harvest Pantry LLC, 325 N. Main St., Piqua — Satisfactory at time of inspection.
311 Drafthouse, 311 N. Main St., Piqua — The temperature of prep units by grill line about 41 degrees: mushrooms, 45 degrees; pineapple, 43 degrees; ham, 54 degrees; tomato, 45 degrees. Keep all TCS food below 41 degrees at all times to prevent bacterial growth. Items moved to walk-in. Will use ice in addition to mechanical refrigerator until repairman arrives. Bacon thawing on counter at room temperature. Thaw items in refrigerator, under running, draining water or while cooking. Product can be above 41 degrees while thawing at room temperature. The microwave interior and hood need to be cleaned.
