Taco Bell, 1234 E. Ash St., Piqua. Violations/comments: The shelf above the fryer needs to be cleaned.
Subway, 1300 E. Ash St., Piqua. Violations/comments: Observed broken food containers/lids. Food contact surfaces must be smooth, durable and easily cleanable. Cracks provide areas for bacterial harborage. Discard.
Red Roof Inn and Suites, 902 Scott Drive, Piqua. Violations/comments: Clean the milk and juice dispensing containers daily after morning use. Remember re-heating is not permitted with current license.
Susie’s Big Dipper, 323 N. Main St., Piqua. Violation/comments: Observed containers of frozen soup available for self service without ingredient labels Food items available for self service must have ingredients listed. Repeat. Observed broken food containers. Food containers must be smooth, durable and intact. Cracks provide areas for bacterial harborage. Dipper well was off but used scoops were being stored. Store scoops clean and dry, in product with handle out or in running, draining water.
Action Works, 8433 N. County Road 25A, Piqua. Violations/comments: Observed lettuce without a date. All TCS foods must be labeled with date opened or prepared. Any food left must be discarded after seven days. Observed noncommercial equipment in kitchen: crockpot, George Foreman grill. All equipment must be commercial NSF or equivalent. The following itms need to be cleaned: microwave interior, pizza cutter, fryer hood.
Dave’s Place, 1106 Fisk St., Piqua. Violations/comments: Provide hand soap for hand sink so hands can be washed, as needed. The hood, ceilings, need to be cleaned. The ceiling in the walk in cooler and kitchen are starting to deteriorate and need repaired or replaced so smooth and easily cleanable.
Sterling House, 1744 W. High St., Piqua. Violations/comments: Observed bulk containers in store room unlabeled. All food items must be labeled unless unmistakable to prevent misuse.
Kids Learning Place, 285 R.M. Davis Parkway, Piqua. Violations/comments: Satisfactory at time of inspection. The corner cabinet is being replaced.
Pretzel Time/Great American Cookie, 987 E. Ash St., Piqua. Violations/comments: The nacho cheese machines need to be washed.

Hobart Arena North West Concessions, 255 Adams St., Troy — No violations were documented at the time of inspection.
Logan’s Roadhouse, 1750 W. Main St., Troy — Critical: Observed butter being held for time marking with proper time being recorded. Observed cutting boards were heavily stained and scored and need to be resurfaced effectively or replaced. Critical: Observed that there was no sanitizing level in the sanitizing water of the three compartment sink in bar area. Person-In-Charge replaced sanitizer in the water during inspection. Critical: Observed food residual build-up on the inside of the microwaves in food prep area. Repeat: Observed food residual on the handles of most of the equipment in food prep area and on the door of the ware-washing machine. Observed dried food residual on the back of slicer blade. Observed grease build-up on the ventilation hoods above fryers. Observed food holding pans being stored inverted before air drying properly in walk-in cooler. Observed forks coming out of the dishwasher stored with food contact surface up. Observed debris build-up underneath back ice machine by restroom area.
Tipp City Eagles, 202 Main St., Tipp City — No violations were documented at the time of inspection.
VFW Pleasant Hill, 7578 Fenner Road, Pleasant Hill — Repeat: At time of inspection no documentation was present showing an employee with level two certification in food protection. Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. Repeat: Observed water standing on walk-in cooler floor near and along the door of the walk-in freezer door. The physical facilities are not being maintained in good repair.
Miami County Jail, 201 W. Main St., Troy — Observed that facility did not have proper test strips for checking sanitizing level in ware-washing machine. Observed that the gaskets on the cooling units had residual build-up on them. Observed shelving units used to hold food in walk-in cooler had residual build-up on them.
Honeybaked, 771 W. Market St., Troy — Observed hot water heater that was not properly installed in the facility as no permit has been taken out for the installation. Observed seal and caulking in disrepair on the lid of the grease trap.
Crystal Room, 845 W. Market St., Troy — Repeat: Observed that facility did not have thermolabels for checking the sanitizing temperature of ware-washing machine. Repeat: Observed ice accumulation underneath condensing unit in the walk-in freezer. Critical, repeat: Observed that during inspection, the water for the ware-washing unit (hot water only) was not reaching a proper temperature for sanitizing. Critical, repeat: Observed cleaned plates and bowls used for food prep that both had food residual present on them at time of inspection.
Walmart, 1801 W. Main St., Troy — Repeat: Observed ice accumulation in the walk-in freezer where ice cream was being stored.
Market Fresh Foods, 301 Troy Pike, Covington — Critical: Observed apples being displayed for customer purchase at facility without protection against contamination. Critical: Observed that the following sandwiches were out of cold holding temperature during inspection: chicken salad sandwiches, 47 F; ham and swiss sandwiches, 46 F; turkey subs, 47 F; ham salad sandwich, 47 F. Person-in-charge threw away the sandwiches listed above during inspection. Critical: Observed packets of ranch dressing that was labelled to be kept in refrigeration held at room temperature. Person-in-charge disposed of packets of the ranch dressing during inspection. Critical: Observed opened bags of pepperoni and a opened bag of shredded cheese with a throw away date of Oct. 30 not properly discarded. Person-in-charge disposed of food during inspection. Critical: Observed donuts in self-service area that did not have ingredient list and an allergen list for each doughnut provided. Person-In-Charge stated that she is currently in the process of getting the ingredient list from the provider.Observed broken handle on the hand-washing sink in the women’s restroom. Observed cutting boards in front of prep cooler that were heavily scratched and stained and need to be repaired effectively or replaced. Critical: Observed dark residual on the inside of the ice machine during inspection. Observed residual build-up on the back of the slicer blade. Observed both hand-washing sinks in the women’s and men’s restrooms did not have hot water available during inspection. Observed no hand cleanser available in the women’s restroom. Observed no hand drying provision in the men’s restroom. Observed broken tiles throughout store that need to be fixed. Observed water dripping from ceiling towards the back of the store in front of packaged soda. Observed missing ceiling tiles throughout store area. Observed ice accumulation coming from the condensing unit in the walk-in freezer. Observed that the cement base and grout underneath the broken tiles throughout facility are gone / in complete disrepair and need to be fixed.
Wendy’s, 825 W. Main St., Troy — Repeat: Observed ice accumulation building up below condensing unit in walk-in freezer.
UTC Aerospace Troy, 101 Waco St., Troy — Critical: Observed cheese (48 F) and cole slaw (50 F) inside of cold holding unit that was stored in unit the day before. Person-In-Charge threw away affected foods during inspection and will keep case closed as often as possible to reduce temperature issues. Observed that the cutting board in cafeteria were scored and holding residual as well as staining. Observed dust build-up on the fan cover inside of food prep area above three compartment sink. Observed leak on piping below three compartment sink.
West Milton Food Mart, 1046 S. Miami St., West Milton — Repeat: Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. Critical, repeat: Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Repeat: Facility was not able to provide written procedures for responding to vomiting or diarrheal events. Repeat: Observed foam and tape being used to block air flow along edge of back door in storage/ware-washing area. Material is not easily cleanable. Repeat: Observed that facility did not have a functioning food thermometer available. Repeat: Observed substance leak coming from behind plastic seal on cappuccino machine. Critical, repeat: Observed accumulation of dried food residual on pizza peel. Critical, repeat: Observed dried food residual on the inside of microwave in front area. Repeat: Observed dust accumulation on the fan covers of the condensing units in the walk-in coolers. Repeat: Observed that the coffee stirrers in front area were not properly protected from contamination. Observed that the piping leading from the dump sink to the mop sink was not constructed properly and shall be fixed by a commercial plumber. Repeat: Observed that there was no floor coving for the floor and wall juncture in storage/ware-washing area. Repeat: Observed missing ceiling tile above hot water heater located above mop sink. Repeat: Observed bare wall behind door leading to storage/ware-washing area. Repeat: Observed section of the wall beside mop sink that is missing. Repeat: Observed cracked floor tile in storage/ware-washing area. Repeat: Observed partial ceiling panel missing above door leading to ware-washing area. Repeat: Observed missing ceiling panels above dump sink in front area that need to be replaced. Repeat: Observed disrepair of the bottom of wall near floor joint. Repeat: Observed damage to the back door that needs to be repaired. Repeat: Observed residual build-up on the ceiling tile above three compartment sink. Repeat Observed black residual located on the walls and ceiling of walk-in drink and beer cooler. Repeat: Observed dark residual on the walls near and behind the three compartment sink.
