From the Piqua Health Department
Cassano’s Inc., 1577 Covington Ave., Piqua. Observed spray bottle on prep area without a label. All food containers must be labeled unless identity is unmistakable to prevent misuse. Corrected. Observed garlic butter container stored on counter at 65 degrees. Marked “keep refrigerated.” All TCS foods must be kept below 41 degrees at all times to prevent bacterial growth.
Dominos Pizza, 120 N. Sunset Drive, Piqua. Provide thermometer for walk in cooler so its temperature can be monitored. Corrected. Organize back store room so non-food equipment items are being kept away from food equipment, i.e., pans stored with motor parts etc. Keep equipment away from possible sources of contamination.
Piqua Marathon, 226 W. Water St., Piqua. Satisfactory at time of inspection.
Burger King, 1408 Covington Ave., Piqua. The following items need to be cleaned: top of oven, warmers, both sets of pop nozzles, walk in cooler, prep unit, microwave interior, fryer area, chest freezer, container with clean food containers. There are missing sections of floor covering by mop sink. Replace so walls are not damaged when floors are mopped. The floors, walls and ceilings need cleaning.
Nicklin Avenue Market, 1601 Nicklin Ave., Piqua. At least one person with management responsibilities must have Level 2 food safety training. Training must be approved by Ohio Department of Health.
From Miami County Public Health:
Walmart, 1801 W. Main St., Troy — Critical: observed bolillo rolls being sold on display without proper ingredient or allergen list, corrected during inspection. Critical: observed the presence of multiple gnats in the donut display case. Critical: observed dried food residual on the top plate inside of microwave, corrected during inspection. Observed that the ingredient/allergen list was hidden for the donut case and not easily accessible. Observed condensation leak above packaged raw chicken from the top of cooling unit on shelving display. Observed water pooling up on corner of wall juncture underneath three compartment sink in the bakery. Observed ice accumulation in the walk-in freezer where ice cream was being stored. Observed ice accumulation on the condensing unit of the walk-in grocery freezer. Observed milk leaking from containers and pooling around the floor of the walk-in cooler, where the area is not being cleaned properly.
Milton-Union Recreation Association, 7640 Milton Potsdam Road, West Milton — No violations were documented at the time of inspection.
Bob Evans Farms, 22 Weller Drive, Tipp City — Customer made a complaint stating that employees are not wearing hair nets in the back kitchen. Upon entering the kitchen area, both employees working in the kitchen were wearing either hats or hair nets. No violations were documented at the time of inspection.
Panera Bread, 1920 W. Main St., Troy — Critical: observed a ware-washing machine visual and audible alarm not working. Observed proofer with a water leak issue, causing water to stand on floor next the proofer. The surfaces of cutting blocks or boards were severely scratched or scored and could not be effectively cleaned and sanitized. This was seen on many cutting boards throughout the facility. Observed water leak near three-compartment sink area on fixture.
Wendy’s, 3 Weller Drive, Tipp City — Observed wiping cloths being stored in a sanitizing solution that was not at the proper concentration in food prep area, corrected during inspection.
McDonald’s, 127 N. Miami St., West Milton — Original Complaint: flies in dining room. Currently working with management to ensure that facility keeps up with pest control. No violations were documented at the time of inspection.
Clark’s Pizzeria, 420 S. Miami St., West Milton — Repeat: observed residual build-up on underneath arm face of large mixer.
Holly’s Cafe & Carryout, 112 N. Main St., Casstown — Critical, repeat: person-In-Charge was not able to provide illness reporting guidelines for employee during inspection. Critical: observed working container of a chemical that was not labelled in ware-washing area, corrected during inspection. Observed the handles of the pizza ovens and microwave that were sticky to the touch. Observed shelving above three compartment sink that is currently rusted. Observed no handwashing signs near handwashing sinks. Observed build up of debris underneath equipment throughout food prep area. Observed build-up of dark mold residual on the piping behind and below the condensing unit as well as the walls of the walk-in cooler where packaged food was stored.
Holly’s Cafe & Carryout, 112 N. Main St., Casstown — No violations were documented at the time of inspection.
Certified, 891 S. Miami St., West Milton — Critical, repeat: the single-slices of pizza and packaged sandwiches on the bottom section of the hot display unit holding between 127F and 132F. Repeat: observed small display hot holding unit not holding proper hot temperature.
Mojo’s Bar & Grill, 109 E. Main St., Troy — Repeat: observed employee drinks being stored on surface near prep sink where raw meat is being prepped. Observed person-in-charge eating and drinking in food prep area multiple times during inspection. Person-In-Charge was not able to show Level 2 Food Safety Certification during inspection. Critical: person-In-Charge was not able to show illness reporting guideline during inspection. Repeat: observed non-handled utensils being stored in container of salad dressing during inspection. Critical: observed food that had been date marked past the 7 day period that was still in the cooler for food use, corrected during inspection. Critical: observed no sanitizing concentration in the three compartment sink out in the bar area. Critical: observed black mold residual on the inside of ice machine in food prep area. Repeat: observed handles on fridge units in food prep area that had food residual on them and were sticky to touch. Critical: observed hand-washing sink in bar area being used to hold trash. Observed a build-up of dirt and debris underneath equipment in the bar area. Observed alcohol spill underneath storage racks in beer cooler.
