Asian Cottage, 761 W. Main St., Troy — Critical violation: Observed food in walk-in cooler that was not properly date marked. Observed residual build-up on handles of the doors on the prep cooler. Observed no towels at the hand washing sink in the food prep area, corrected during inspection. Observed tiles being uplifted underneath food shelving unit towards the back of the facility.
Tall Tales Inn, 6 W. Pike St., Laura — Critical, repeat violation: Observed food in walk-in cooler that was uncovered, and was not properly protected from contamination by packaging. Repeat observation: Observed pizza spatulas sitting on top of unclean over. Repeat observation: Observed residential reach in freezer in kitchen. Equipment is not approved by a recognized testing agency. Repeat observation: Observed white chest freezer by the pizza oven where the gasket is coming apart from the freezer lid. Repeat observation: The surface of the cutting board on the prep cooler is severely scratched or scored and can not be effectively cleaned and sanitized. Repeat observation: Observed re-use of single-service salad dressing containers to store iced tea. Repeat observation: Observed food debris in the bottom of reach-in two door freezer by the walk-in cooler. Non-food contact surfaces of equipment are dirty. Repeat observation: Observed a build-up of grease and food debris on all cooking equipment on grill line. Non-food contact surfaces of equipment are dirty. Repeat observation: Observed no service sink or curbed mop sink facility as required. Repeat observation: The floor and wall junctures are not properly coved or closed in the dry stock area.
Troy Christian High School, 700 S. Dorset Road, Troy — Observed ice machine with dust on upper edge of metal flap.
Hook Elementary School, 729 W. Trade Square, Troy — Observed food in packaging that does not appear to protect it from contamination, observed dented can of applesauce in dry stock area. Observed paper towel dispenser in kitchen restroom that did not have the protective cover on.
Concord Elementary School, 3145 State Rt. 718, Troy — Observed residual build-up on the floor near the baseboard underneath dishwasher and counters.
Big Mike’s Gas N Go, 8561 State Rt. 201, Tipp City — Repeat observation: Bulk food display for customer self-service was not properly labeled. Observed brownies, pretzel bites, and donuts without ingredient labeling or allergen declarations.
Harrison’s, 106 E. Main St., Tipp City — Observed water accumulating in the bottom of reach in bar cooler on the left hand side of the bar. Critical violation: Observed build-up residual inside of soda gun at the bar. Observed a build-up of dust and food debris on shelving where clean plates are kept on cooking line, and also on shelf used for spices. Repeat observation: Observed food residual and build-up on wire shelving in walk-in cooler. Repeat observation: Observed standing water underneath the server station hand sink by the bar. Repeat observation: Observed no hand washing sign(s) posted at hand washing sinks in restrooms. Repeat observation: Observed a build-up of dirt and debris underneath and behind ice machine. Repeat observation: Observed food residual and debris under shelving in walk-in cooler. Observed black residual on FRP wall in the kitchen, closest to the door leading to the outside. Observation: Observed mops being dried using an unapproved method in second floor kitchen.
Mr. Deals Supermarket, 689 S. Miami St., West Milton — Critical violation: Observed eggs in carton stored directly on top of Ready To Eat foods inside of walk-in cooler behind deli department, corrected during inspection. The surfaces of cutting boards inside of the deli area were observed to have been heavily scored. he plumbing system was not properly repaired. Observed leak on the faucet above the sink. Observed debris built up underneath shelving unit in the walk-in freezer by the meat department. Observed build-up of debris underneath the shelving units inside of the walk-in cooler near the produce area.

Observed ice scoop sitting on top of ice machine, corrected during inspection. Observed hand sink faucet that is loose and pulling away from the sink, hand sink is the one closest to the cook line. Observed no hand washing sign(s) posted at hand washing sink(s) in restrooms near the front entrance. Observed cracked tiles. and areas of low grout throughout kitchen.
The Caroline, 5 S. Market St., Troy — Observed employee drink sitting on table where food prep was being done, corrected during inspection. Observed food in packaging that does not appear to protect it from contamination, corrected during inspection. Found dented cans in storage area. Observed ice scoop being stored in the ice in ice box towards the front of the food prep area, corrected during inspection. Repeat observation: Observed paper towels being stored in the men and women’s bathroom where they are exposed to splash and other contamination. Repeat observation: Observed no towels inside of the hand towel dispenser above the hand washing sink inside of food prep area.
ReU Juicery & Organic Kitchen, 3 E. Main St., Troy — Observed reach-in freezer that was not functioning properly. Freezer had a temperature of 55 degrees. Observed debris and older food residual underneath of electrical box and in hard to clean areas of the juice press. Observed employee personal items such as cell phones and jackets in an undesignated area in the kitchen.

The ice cream scoops were stored used in dipper well that was not running. Utensils must be stored clean and dry; in product with handle extended or in a dipper well that is running and draining.
Charley’s Steakery, 987 E. Ash St., Piqua — The following items need to be cleaned: cabinet interiors, ice machine, food processor, slicers and tubs.
Several lights have burnt out in back prep area. Replace so there is adequate lighting.
Remove items from the facility that are not needed for its operation to aid cleaning, i.e. painting supplies, helium tank, etc. Repeat issue.

Piqua High School, 1 Indian Trail, Piqua — Observed yogurt cups and container of butter kept at room temperature that are marked “Keep Refrigerated.” All TCS foods must be kept below 41 degrees at all times to prevent bacterial growth. Discard these items and display safely in future.
Observed signs of mice in store room. Continue to use traps, etc., to try and eliminate issue. New shelving ordered. Un-used items being removed. Continue to address and monitor until corrected.
Piqua Manor, 1840 W. High St., Piqua — The ceiling in the dish room is damaged and needs to be repaired.
KFC No. 3, 1251 E. Ash St., Piqua — The hood and the interior of the walk-in need to be cleaned.

Wendy’s Old Fashioned Hamburgers, 1221 E. Ash St., Piqua — Observed cracked food containers/lids. Food containers should be smooth, durable and intact. Cracks provide areas for bacterial harborage.
The pop nozzle in front counter needs cleaned.
Twin Dragon, 1598 Covington Ave., Piqua — Observed raw chicken stored above cooked chicken in reach-in cooler. Keep all raw meats below cooked items to prevent possible cross-contamination. Also, raw chicken above cooked in walk-in cooler. Corrected.
Cooked General Tso’s chicken at room temperature 94-80 degrees. Keep all TCS foods below 41 degrees or above 135 degrees to prevent bacterial growth. Item moved to walk-in.
Observed food items in prep cooler above 41 degrees. Corn 48 degrees; mushrooms, 48 degrees. Keep all TCS foods below 41 degrees at all times to prevent bacterial growth. Cooler turned down.
Raw frozen meat thawing in stagnant water. Do not thaw in stagnant water. Safely thaw in refrigerator, while cooking, under running, draining water, or in microwave if cooked immediately.

Disposable grocery bags being used to store food in walk-in freezer. Do not use these items to store food. They cannot be properly cleaned.
