Aug. 1

Genesis Healthcare Troy Center, 512 Crescent Dr., Troy — Equipment and/or components are not maintained in good working order. The walk-in freezer has accumulated ice and frost build up around the door and the compressor area. Critical violation: equipment food-contact surfaces or utensils are dirty. Observed food debris and stickiness on the can opener. Observed mildew growth inside the ice machine. Corrected during inspection. Observed mops being dried using an unapproved method with mop head on the floor. Corrected during inspection. Critical violation: observed the presence of live insects, rodents, and other pests. Observed gnats in the janitors closet. Observed ants on floor in the kitchen.

Aug. 2

Domino’s Pizza, 937 W. Main St., Tipp City — Cooking equipment or pan surfaces are dirty. Employee states they do not wash, rinse, or sanitize the pizza pans, only run them through the pizza oven. Pans must be washed, rinsed, and sanitized to prevent accumulation of food debris. Equipment food-contact surfaces and utensils were not being sanitized especially the pizza pans. Observed mops being dried using an unapproved method, mop holder is broken. Repair or install a new holder.

The Golden Leaf Tea and Herb Co. Ltd., 128 E. Main St., Tipp City — Critical violation: refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked. Lunch meat taken out of freezer was not dated properly with current date. Corrected during inspection.

Aug. 3

Community Bible Church Little Lighthouse, 1427 W. Main St. Tipp City — No violations observed.

Aug. 4

Grounds for Pleasure Coffeehouse, 115 E Main ST, Tipp City — Non-food contact surfaces of equipment are dirty. Observed slime build up inside ice machine on panel and tubing. Clean and sanitize the inside of the ice machine. The light intensity in an area where an employee was working with food, utensils or equipment was less than fifty foot candles. The food/coffee prep area did not have the required intensity of lighting. Observed no dressing rooms or lockers for employees. Employees purse and clothing were sitting directly on top of cutting board.

Holiday Inn Express, 1100 W Main ST, Tipp City — Facility was clean and in good order.

Aug. 11

Piqua City Pool, South Street, Piqua — Satisfactory at time of inspection. Use test strips to make sure enough sanitizer is being used.


Piqua DQ Grill & Chill, 1288 E. Ash St., Piqua — Observed food containers without lids on prep table. Provide lids for food containers to prevent potential contamination. Observed cracked food containers. All food containers shall be smooth, durable and intact. Cracks provide areas for bacterial harborage. There were pieces of clean equipment and food containers that needed a more thorough cleaning. Hand sink by prep table not working. Repair promptly so hands can be easily washed.
