Restaurant inspections are performed in the county by Miami County Public Health, except in Piqua, which has its own health department.

True North No. 715, 600 South St., Piqua — At least one person in charge must have a Level II food safety certificate from the Ohio Department of Health. Obtain needed certification.

Aug. 21

Old Mason Winery and Vineyard, 4199 S. Iddings Road, West Milton — No violations were documented at the time of inspection.

Aug. 22

Caldwell House, 2900 Corporate Drive, Troy — Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. Critical, repeat: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Observed refrigerator in food prep area freezing food on left side of fridge. Repeat: observed handle on the first fridge door falling in disrepair. Critical, repeat: observed food residual build-up on the top of the inside of microwave.

Burger King, 1829 W. Main St., Troy — Observed food residual on the handles of the microwaves and ovens. Observed ice accumulation on the piping behind condenser unit inside of walk-in freezer. Repeat: observed debris build up in the corner behind soda box rack in the back of food prep area.

Ducky’s, 100 W. Market St., Troy — Observed food storage container that is in use and not NSF approved in food prep area.

Aug. 23

Meijer Distribution, 4200 S. County Road 25-A, Tipp City — No violations were documented at the time of inspection. Equipment temperature good at time of inspection. Health timers worked properly on equipment. Facility is clean.

Milton-Union Schools, 7610 Milton Potsdam Road, West Milton — Observed food employee washing hands prior to donning single use gloves and changing gloves when needed. Person-in-charge demonstrated good knowledge about food cooking and holding temperatures. Observed hot foods being held at 135°F or above; cold foods being held at 41°F or below. Observed leak with the piping on the dishwasher sprayer beside dishwasher.

Subway, 695 S. Miami St., West Milton — No violations were documented at the time of inspection.

La Fiesta Express, 1133 W. Main St., Troy — Critical: observed employee using the three compartment sink to wash hands. Manager informed the employee to only use the hand washing sink when washing hands. Observed rack used in holding spices that is not approved for holding foods. A test kit could not be provided during inspection. Observed no hand washing sign(s) posted at hand washing sink(s) in the food prep area.

McDonald’s, 127 N. Miami St., West Milton — Critical: observed breakfast burrito scrambled egg mix sitting out at 55°F, corrected during inspection. The burrito cooked egg mix was sitting out on a prep table that was left out during burrito prep. The food employee left the burrito prep area to work in another area. Manager disposed of the burrito mix. Critical: observed raw shell eggs sitting out at room temperature on shelf above the egg cooking area. Manager threw the eggs away. Critical: observed improper use of time as a public health control for up to four hours. Four containers of unrefrigerated lettuce were not time marked on prep sandwich line. Observed condensation water dripping on to the floor from the walk-in cooler condenser draining line. Ice has built up on the lines and is causing pooling water on the floor near the walk-in cooler door. Observed metal sandwich wrapper paper tray holders on prep line with food residual build-up. Observed unclean gaskets on small reach-in coolers. Examples are McCafe Drink coolers and prep cooler. Observed prep sink unclean with food residual coating. Manager stated the prep sink is not a food contact surface. But the sink is used for draining pickle and onion containers. Clean this sink properly. Observed the leaking water on a pipe in the facility backflow prevention area or water main line area for the building. The plumbing system was not properly repaired. Observed dumpster enclosure pad area being used for storage of unused and unnecessary items. Repeat: observed areas of low grout on the floor throughout the kitchen; water and food debris are collecting in these areas. Repeat: wall between three-compartment sink and water heater is loose/unstable and missing baseboard. Observed build-up on floor in small chemical storage closet room and unclean bread racks (that are being used as platforms for clean items). Observed a build-up of residual and debris under equipment on floor throughout the food service. Examples are under grill/cooking equipment, under ice machine, and drink stations. Critical: observed the presence mouse droppings at back door area. Mouse droppings observed in no other location in the food service. Critical: observed the presence of live gnats at the McCafe coffee/drink station area and customer self serve drink and ketchup area (especially inside the cabinet near the tomato dispensing bag holder).

Pizza Hut, 1480 W. Main St., Troy — Repeat: observed gasket on drink cooler coming off of door in front of food prep area. Repeat: observed caulking on the back of the three compartment sink that is coming off. This caulking needs to be repaired. Repeat: observed a build-up of residual on the floor next to the refrigerator by the pizza ingredient prep cooler.

Aug. 24

He5 Asian Bistro, 121 E Staunton Road, Troy — Critical, repeat: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Repeat: person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. Critical: the person in charge did not ensure that persons unnecessary to the operation were not present in food prep or storage areas. Observed child that is in the facility running into food prep and ware washing areas. Observed multiple dented cans in the back storage area. Critical Observed employee drink can being stored on vegetables inside of prep cooler. Manager removed can from vegetables and threw away affected foods from prep cooler in front of inspector. Observed non-handled bowls and cups being stored directly in contact with salt, sugar and flour in back storage area. Repeat: time as a public health control was being used for mixed made in-house sauces without the proper written procedures. Salt, sugar and flour stored in non-NSF container (blue sterilite containers) in back storage area on racks. Observed no hand washing signage posted on the hand washing sink in the food prep area. Repeat: observed sofa/mattress combo inside very back area of facility in storage room area.

Aug. 25

Troy Country Club Snack Bar, 1830 Peters Road, Troy — No violations were documented at the time of inspection.

Cassano’s, 1201 Experiment Farm Road, Troy — Repeat: in-use utensils are improperly stored. Observed non-handled bowls stored in ingredients in pizza food prep cooler. Person-In-Charge removed bowls from prep cooler during inspection. Observed employee clothing on the rack of the dry food storage area. PIC removed clothing and placed them in the appropriate locker during inspection. Observed panel in front of ventilation unit in pizza prep cooler coming off of equipment. Observed ice accumulation on the piping behind the condensing unit inside of the walk-in freezer.
