May 15

Board & Brush, 816 W. Main St., Tipp City — No violations observed.

Duke Park Concession Stand, 1670 Troy-Sidney Road, Troy — Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events.

Milton-Union Rec Concession Stand, 7640 Milton-Potsdam Road, West Milton — Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. Critical violation: temperature controlled for safety foods were maintaining around 45 degrees Fahrenheit, corrected during inspection. TCS foods were not being held at the proper temperature. Observed ice being used to cold hold temperature controlled for safety foods. Observed inadequate equipment for cold holding food.

Tipp City Middle School, 55 N. Hyatt St., Tipp City — Critical violation: observed raw eggs in carton being stored above ready-to-eat foods in the walk-in cooler, corrected during inspection. Critical violation: observed plastic cups of bacon bits stored inside the main storage bin of bacon bits, corrected during inspection. Observed a lot of ice accumulation on the piping on the inside of the walk-in freezer.

May 16

Miami East High School, 3925 N. State Route 589, Casstown — Observed dented can in the back storage area of the facility. Observed build-up of dark residual on the back of walk-in cooler fan. Observation: Observed dust build-up on the fan casings in walk-in cooler. Observed large amounts of dust build-up behind Freeze drink machines in the sales floor area.

Speedway, 1894 W. Main St., Troy — Observed employee drinks and personal items on top of food prep table, corrected during inspection. Observed dried residual on the handles of the microwave and the oven. Observed an improperly cleaned storage area for refuse, recyclables, or returnables. Observed debris build-up in the back section of the dumpster area.

May 17

Budget Inn of Troy, 1330 Archer Drive, Troy — Critical, repeat violation: TCS foods were not being held at the proper temperature, corrected during inspection. Observed milk being stored at 48 F. There is no test kit available for measuring the concentration of the sanitizer. Observed coffee stirrers being stored in breakfast area without proper protection. Observed no hand washing sign in bathroom. Observed hole in wall underneath hand washing sink.

Dollar General, 1931 Covington Ave., Piqua — A working food thermometer was not readily accessible, in upright display cooler. Observed paper towels not in a paper towel holder in restrooms.

Improved Order Of Redmen, 103 1/2 E. Main St., Troy — Observed dried residual built up inside of pizza oven below rack. Observed no supply of hand soap by handwashing sink behind bar.

Shell, 1015 W. Main St., Tipp City — Observed that the back of the dumpster area had trash bags and debris behind dumpster that needs to be cleaned out.

Troy Christian Elementary School, 1586 McKaig Ave., Troy — No violations upon inspection.

Troy Christian High School, 700 S. Dorset Road, Troy — There is no test kit available for measuring the concentration of the sanitizer. critical violation. Observed dried residual on top of microwave in food prep area. Observed holes in walls underneath both prep sink and three compartment sink.

Troy Rec, 11 N. Market St., Troy — Food operation in good condition at time of inspection.

Troy Senior Citizens Center, 134 N. Market St., Troy — Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health.

May 18

Al’s BP Oil Service, 6 E. Broadway, Covington — Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Cold holding fridge in back area did not have a proper temperature measuring device inside of it. Repeat observation: observed drink cups and lids in plastic stored underneath drainage piping in cabinet under drink machine.

BP, 1590 W. Main St., Troy — Observed sheets of cardboard paper stored on ground behind dumpster.

Izzy’s Drive Thru, 7500 State Route 571, West Milton — Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. REPEAT OBSERVATION Observed fishing bait being stored in cooler with packaged beverages. Observed paper towels standing by themselves on table next to toilet without protection from contamination. Observed no hand drying provision by the handwashing sink by the restroom entrance. Observed ice build-up underneath the cooler fan in the walk-in beer cooler dripping condensation on boxes and on floor. The physical facilities are not being maintained in good repair.

May 19

Fulton Farms Market, 2393 State Route 202, Troy — Critical violation: person-in-charge did not show in a verifiable manner the employees’ responsibility to report information about their health during inspection. Critical violation: observed raw shell eggs being stored above packaged pies in fridge unit on sales floor, corrected during inspection. Observed no towels or hand drying device at the hand washing sink behind ice cream bar. Observed hole in wall beside hand sink in ice cream parlor.

Tipp City Preschool Learning Center, 1221 W. Main St., Tipp City — Observed sealed case of disinfection wipes stored on top of wrapped flour tortillas in kitchen, corrected during inspection.
