Health inspections


Restaurant inspections are performed in the county by Miami County Public Health, except in Piqua, which has its own health department. Miami County Public Health can be reached at (937) 573-3500, by email at info@miamicountyhealth.net or on the website at www.miamicountyhealth.net.

These inspection reports were provided by Miami County Public Health.

May 1

CVS Pharmacy, 14 W. Main St., Tipp City — No violations observed at time of inspection. Facility has required written procedures.

Forest Elementary, 413 E. Canal St., Troy — New manager is going to be taking the Level 2 serve safe class and exam this summer. No violations upon inspection.

Meijer, 1990 W. Main St., Troy — Observed box of frozen food stored on the ground in the walk-in cooler, corrected during inspection. Observed an improperly cleaned trash area behind the facility.

Taco Bell, 85 W. Main St., Tipp City — Critical violation: observed that the concentration of the sanitizing level was less than required by the manufacturer’s labeling in the three compartment sink, corrected during inspection. Critical violation: observed residual build-up on the side of the ice shoot on the drink dispenser in the dining area, corrected during inspection. Observed drink nozzles on drink machine in the dining area to have built-up residual on them. Handles on the hot holding units near the cook line were observed to have residual on them. Observed ice scoop being stored on top of the ice machine, resting directly on the top of the machine. Observed panel pulling from the ceiling where water pipes are behind cooling fans inside of walk-in cooler. Repeat observation: observed debris and build-up underneath shelving units in the food prep area and the back area where storage is at.

May 2

Miami County Locally Grown Virtual Farmers Market, 16 W. Market St., Troy — Market was in great operation at time of inspection.

May 4

Igloo Drive Thru, 45 W. Main St., Tipp City — Critical violation: person-in-charge was not able to provide employee sickness verification form during the inspection. Observed bottom vent plates underneath the drink machines falling apart. Observed dust residual build-up on fan blades and blade covers inside of walk-in cooler. Observed no hand washing sign inside of employee bathroom. Observed a large portion of the door badly damaged leading to the employee bathroom.

Kountry Kruise, 7235 US Route 36, Fletcher — Critical violation: person-in-charge did not have employee sickness verification form. Observed accumulation of dust residual on the fan casings inside of the walk-in cooler. Facility could not show employee sickness verification form. Send water sample for the facility to the Miami County Health Department as soon as possible.

Menards, 75 Weller Dr., Tipp City — Observed no hand washing signage in the family restroom. Facility had all proper forms.

Walgreens, 20 W. Market St., Troy — Observed soup cans in aisle that were dented, corrected during inspection. Observed debris build-up underneath cardboard recycle bin area behind store.

May 5

Action GBW, 201 Staunton Road, Troy — Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Observed vent plates underneath drink coolers that were falling apart. Observed no hand washing sign in the bathroom area of the facility.

Dollar General, 305 S. Main St., Pleasant Hill — Observed dented cans in the aisles of the facility, corrected during inspeciton. Observed small leak coming from the faucet above the mop sink in the back storage area. Observed inappropriately cleaned refuse area outside the facility.

Whole Health For The Whole Family, 22 S. Weston Road, Troy — Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health.
