DJ Pizza Alley, 18 S. Second St., Tipp City — Observed utensil with handle inside of flour bin. Critical violation: observed steak inside of the walk-in cooler that was not properly date marked. Repeat observation: observed reach-in freezer in the outside shed area that is not approved by a recognized testing agency. Observed no hand washing sign(s) posted at hand washing sink(s) used by employees. Observed food debris underneath shelving unit and towards the corner behind the pizza oven.
Applebees Neighborhood Grill & Bar, 1759 W. Main St., Troy — Observed hand sink where caulking was coming apart from the wall leaving a gap between the sink and wall. Observed ice build-up in walk-in freezer, possible condenser issue. Critical, repeat violation: Observed microwaves with food residual on top interior portion, and on sidewalls, manager had employee clean microwaves at time of inspection. Repeat observation: found a black residual build-up on gaskets in prep coolers behind the cooking line. Repeat observation: observed a sticky residual build-up on handles of equipment throughout facility, and along prep cooler doors by the grill. Repeat observation: observed a water leak at the mop sink faucet. Observed a build-up of dirt and debris on floor throughout kitchen, especially in hard to clean areas such as in-between and underneath cooking equipment, behind ice machine, and under the prep sink.
Bakehouse Bread Company, 317 SW Public Square, Troy — Observed used towel on clean dishes at three-compartment sink. Repeat observation: observed restroom that did not have a tight-fitting, self-closing door. Observed no hand washing sign posted at hand washing sink in rest-room used by employees. Temperatures were within required ranges. Observed good hand washing and glove usage.
Crystal Room, 845 W. Market St., Troy — Miscellaneous sources of contamination observed: observed meat cutting equipment being stored on sealed box of shrimp inside of walk-in freezer, corrected during inspection. Critical violation: food contact surfaces were not easily cleanable, or not designed/constructed properly. Observed pegboard being used to hold cooking utensils above grill inside main catering kitchen. Cleaned and sanitized equipment, utensils, laundered linens, or single-service articles or single-use articles were stored in a prohibited area. Observed a dirty tray being stored on top of clean plates and trays in back storage area in dish washing room, corrected during inspection.
Newton Local School, 201 N. Long St., Pleasant Hill — Observed employee drink sitting on handwashing sink and hand-drying provision.
The Refinery, 302 N. Miami Ave., Bradford —Observed reach in freezer in back storage room with a large amount of ice build-up. Observed a sticky residual on equipment handles in facility, specifically on the microwave and refrigeration handles. Observed restrooms that did not have a tight-fitting, self-closing door.
Tim Horton’s, 700 W. Main St., Troy — No violations at time of inspection.
Tipp O’ the Town, 1150 W. Main St., Tipp City — Observed clear plastic cup without handle inside bin of cubed ham. Repeat observation: observed wiping cloths being stored in a pre-soak solution instead of a sanitizing solution. Critical, repeat violation: residual build-up on the ice chute of the soda machine at the waitress station. Repeat observation: no hand washing sign posted at the sink in the men’s restroom. Observed residual build-up in the top corner of the walk-in cooler. Repeat observation: observed a build-up of dirt and debris underneath shelving units inside of walk-in freezer.
Troy Meat Shop, 502 Garfield St., Troy — Repeat observation: observed bags of frozen onion rings in reach-in freezer that did not have any labeling or ingredient listing on the bag. Observed casing for the cooling fan in the reach-in freezer separating from the fan. Observed dust build-up on the fan cases inside of walk-in cooler. Observed a hole in the brick wall next to the three-compartment sink. Repeat observation: observed hole in the wall underneath the hand washing sink. Observed a build-up of dirt and debris underneath the display meat cooler.
36 Skate Club, 845 W. U.S. Route 36, Piqua —No violations observed at time of inspection. Facility has required written procedures, and an employee with level 2 training in food safety. Keep up the good work.
The Brewery, 111 W. Water St., Troy — Observed ice scoop being stored improperly on top of ice machine. Critical, repeat violation: observed sugar and residual build-up on the end of the drink nozzle behind bar area. Repeat observation: observed no hand washing signs posted at the hand washing sink behind bar or in the men’s and women’s rest-rooms. Observed soda syrup and debris on the floor below soda boxes in closet area. Observed dirt build-up on the mop sink.
Riverside School, 1625 N. Troy-Sidney Rd., Troy — Observed no towels or hand drying device at the hand washing sink inside of the food prep area, corrected during inspection. Observed large hole in wall underneath dish washing sink.
Skippers Tavern, 101 N. Miami St., West Milton — Critical, repeat violation: observed broccoli cheese soup in walk-in cooler with a date of Jan. 24, corrected during inspection. Observed walk-in cooler with a temperature of 50 degrees, corrected during inspection. Observed dusty shelf above prep cooler near three compartment sink. Observed handles on reach in freezers in kitchen with a sticky residual build-up. Observed no hand washing signs posted at hand washing sink in men’s restroom used by employees.
Springcreek Primary School, 145 E. U.S. Route 36, Piqua — No violations at time of inspection.
End Zone Sports Lounge, 601 E. Broadway St., Covington — Critical violation: observed food in contact with an unclean surface or linens, bowls of salad were stored directly on other bowls of salad with the bottoms of the bowls touching the salad beneath. Critical violation: observed pastrami in upright freezer and roast beef in the walk-in freezer that was not properly date-marked.Observed the handle doors on the upright coolers missing the handle guards. Observed residual on the handles of the upright freezer. critical violation. Hand washing sink is being used for purposes other than hand washing. Observed ice being dumped inside of the hand washing sink behind bar counter.
Hampton Inn, 45 Troy Town Drive, Troy —Observed juice dispensing machine with a sticky residual build-up underneath where nozzles are.
Marsh Supermarket, 982 N. Market St., Troy — Critical violation: observed chocolate turtles and chocolate covered nuts that were being put together at the store without proper labeling and allergen list. Critical violation: observed calcium and residual dripping from the underside of shelving unit in the produce area where uncovered broccoli and cauliflower were being sold. Observed no hand washing signs posted by the hand washing sinks in both the meat department and the deli department. Observed a build-up of debris underneath the shelving inside of the walk-in freezer unit towards the back of the building on the far left.
Honeybaked, 771 W. Market St., Troy — Observed residential chest freezer being used to freeze bread. Chest freezer must be of commercial grade. Observed excessive ice build-up on the piping inside of the walk-in freezer.
St. Patrick Parish Center, 444 E. Water St., Troy — Repeat observation: observed a build-up of dirt and debris underneath and in-between cooking equipment. Previous violations from inspection Oct. 27, 2016, have been resolved. Facility has required handouts and level 2 training. Keep up the good work.
