Health inspections

July 10

West Central Juvenile Center, 2044 County Road 25-A, Troy — Observed food in packaging that does not appear to protect it from contamination. Observed dented can in back storage area. Observed ice accumulation on the back of the walk-in freezer fan. Observed leak on faucet above three compartment sink and on the piping below it.

July 11

Community Bible Church Little Lighthouse, 1427 W. Main St., Tipp City — Critical violation: observed food that was not properly protected from contamination by separation, packaging, and segregation. Observed eggs stored improperly inside of refrigerator, corrected during inspection. Observed no hand washing signs inside of bathrooms.

July 13

Koester Pavilion, 3232 N. County Road 25-A, Troy — Critical violation: person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Observed wiping cloths being stored in an improper solution that had no sanitizing levels. The plumbing system was not properly maintained. Observed leak on the faucet above three compartment sink.

July 18

Pizza Hut, 1480 W. Main St., Troy — Observed gasket on drink cooler coming off of door in front of food prep area. Observed caulking on the back of the three compartment sink that is coming off. This caulking needs to be repaired. Observed water leak on the tile ceiling towards the back of the food prep area. Tiles are bowing downwards. Leak is currently contained by the use of a mop bucket. Observed a build- up of residual on the floor next to the refrigerator by the pizza ingredient prep cooler.

July 19

Los Pitayos, 2317 W. Main St., Troy — Critical violation: person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. Critical violation: observed single-use gloves being used improperly. Observed employee walking from dish washing area to do food prep without changing gloves, corrected during inspection. Nonfood-contact surfaces are not designed and constructed to allow easy cleaning or maintenance. Observed rust on the racks being used to store foods in walk-in cooler. Repeat observation: non-food contact surfaces of equipment are dirty. Observed food residual on handles of equipment both in the kitchen area and in the bar area. Observed leak on faucet of the vegetable prep sink and above the three compartment sink. Observed no hand washing sign posted at hand washing sink in the food prep area. critical violation. Repeat observation: observed working container of poisonous or toxic chemicals in side storage area that was not properly labeled with the name of the material, corrected during inspection.

July 20

Meijer, 1900 W. Main St., Troy — Critical violation: in the deli area, the raw chicken walk-in cooler was observed with raw chicken breasts uncovered. In the deli area cold holding unit against the wall adjacent to the blast chiller, a pan of cooked chicken breasts were observed uncovered. Beneath the beverage fountain in the front section of the store, boxes of soda bags were observed being stored on the floor. Critical violation: on the sales floor in the produce area, multiple chicken sandwiches (packaged Jimmy Dean and Meijer Spicy Chicken sandwiches) in the hot holding unit were observed holding between 118 F and 124 F. n the deli area hot display unit, one pan of pulled pork was observed holding at 127 F. In the bakery area, the bagels were observed without labels. Critical violation: a fruits and vegetables cart in the produce prep walk-in cooler was observed with rust build-up. The following walk-in freezers were observed with ice build-up on the compressor units: in the back storage area, the seafood walk-in freezer and the west side walk-in freezer, the deli walk-in freezer. On the sales floor in the produce area, the hot holding unit was observed holding chicken sandwiches between 118 F and 124 F. Hot holding must be maintained at or above 135 F. In the back storage area, the west side walk-in freezer was observed with gaps between the sliding doors and door threshold resulting in standing water and ice build-up. The following walk-in coolers were observed with fan covers with dirt and dust debris build-up: the deli raw chicken walk-in cooler, the produce storage walk-in cooler, the produce holding walk-in cooler, the produce prep walk-in cooler, the meat cutting room, the raw meat storage walk-in cooler. Critical violation: in the produce holding and prep walk-in coolers, food debris build-up was observed indicating use as a prep sink. Also, upon discussion with an employee, confirmation was made, the hand sinks were being used to wash vegetables. Observation: plumbing beneath the sanitizing compartment of the deli area three compartment sink on the northwest side (near the deli slicing area) was observed leaking water. In the raw meat and seafood area, the fixtures on both three compartment sinks were observed leaking water.

July 25

Charleys Steakery, 987 E. Ash St., Piqua. Observed less than 200 ppm quat being used to wash, rinse and sanitize utensils. Only 100 ppm being dispensed. Must use at least 200 ppm of quat to properly sanitize. Repair called. The following items need to be cleaned: fans, “clean” food containers; shelving; fryer area. Wiping cloth stored in 100 ppm quat. Must store wiping cloths in sanitizer between uses to prevent bacterial growth. Observed soiled food uniform on store rack. These must be clean. Damaged seal on the freezer door. Repair seal on walk in freezer door. Handle for uncooked frier dispenser stored in product. Keep dispensing utensil handles extended so not in product. Corrected. The drain for the front hand sink was not working properly. Repair.

Hersheys, 987 E. Ash St., Piqua. The blender and the dipper well need to be cleaned. The floor and the cabinet need to be cleaned.
